What is the Dementia Connect Line?

The Dementia Connect Line is a direct phone line for anyone in our region seeking compassionate support, practical information, and pathways to local dementia services, with no referral required.

Dedicated Weekly Hours

  • Monday: 10:00 AM – 12:00 PM
  • Tuesday: 12:30 PM – 2:30 PM
  • Wednesday: 10:00 AM – 12:00 PM

Hours & Closures

Our Dementia Connect Line will be closed December 24th, 29th, 30th, and 31st for the Christmas Holidays and will reopen on January 5th, 2026.

You’ll connect with Sarah Daniels, First Link® Care Navigator, a trusted point of contact who understands dementia and the local care system.

After-hours support

If you need to reach us outside of the dedicated hours, please leave your name, phone number, and a brief reason for your call. You may also call the office to self refer and set up an in depth appointment for a later date. For urgent safety concerns, call 911 or visit your nearest emergency department.
