The Dementia Connect Line is available:

Monday to Friday
9:00 AM – 4:30 PM

You’ll be connected with one of our First Link® Care Navigators, trained in dementia support and guidance.

If we’re assisting another caller, please leave a voicemail with your name and phone number if you feel comfortable doing so.
If not, you’re always welcome to try again.

All calls are handled with care and confidentiality.

After-Hours Support

If you contact us outside of these hours, please leave your name, phone number, and a brief reason for your call.

You may also call the line to self-refer and schedule a more in-depth appointment at a later date.

For urgent safety concerns, please call 911 or visit your nearest emergency department.
