How First Link works

Not sure how First Link works? Read this step by step guide to implementing the program in your office:

  1. As soon as the diagnosis is made, a health-care professional such as yourself asks the person with dementia and his or her family for permission to refer them to First Link.
  2. Once consent is given, the health-care professional completes a referral form and sends it to the Alzheimer Society First Link team.
  3. Within four weeks, a First Link support counsellor calls the person with dementia or a caregiver to provide information, assess needs and connect them directly to information and support services in their community. Those with diagnoses and their families can also call First Link directly.

First Link offers a single point of entry to appropriate health-care information and referral to other services available locally, such as medications, activity programs, education and life planning.
